SIMS (Student Information Management System)
Security Access Request Forms
The two types of SIMS forms available are described below.
PLEASE READ the general instructions for
completing these forms (college of engineering
employees only):
- Complete the top section of the form
(everything above the "OPTIONAL" section surrounded
by dashed lines)
NOTE: 3/1/05 - The UIN is required. Forms that include SSN
instead of UIN will no longer be processed, but will be
returned.
- Information for line 4
• Campus: CS (college station)
• College: EN (engineering)
• Department: XXXX (use departmental 4 letter abbreviation)
• Primary Area: AC (academic)
- If you need access to grades and test scores, you must
check "YES" in the gray shaded box and provide
a reason. Then check the back of the form and cross out
any sensitive screens that you DON'T want.
- You should have your department head sign the form under
the SCREEN APPROVAL AUTHORITY SIGNATURES section.
- Return the form to the Engineering Student Services and
Academic Programs Office (see address below). We sign/date
the form as the PRIMARY AUTHORIZING SIGNATURE for the college
and forward it to SIMS.
- The SIMS office will call to schedule any necessary training
sessions (they will call the phone number provided at the
top of the form). As of April, 2007, all SIMS training classes
will be held in the SIMS Training Center classrooms on the
second floor of the General Services Complex (GSC) located
at the north end of Agronomy Road.
- It may be helpful to keep a copy of the form for your
records - the available/requested SIMS screens are listed
on the back of the form.
